Eligibility

Open to anyone age 18-59 who resides in an Illinois nursing facility, has a severe disability which will last for 12 months or the duration of life, has physician's approval of plan of care, has no more than $17,500 in non-exempt assets, and receives or applies for Medicaid benefits
