Contact Capstone for complete information; intake forms may be printed from the website and completed prior to setting up the first appointment; initial intake appointment cannot be scheduled until intake forms are completed and returned to the office

Accepts private insurance, Medicare, Medicaid, CPC County approval or private pay, Jasper and Marion County residents may be eligible to pay fees based on a sliding fee scale
