Eligibility

Open to those age 18 and older who are a US citizen or legal resident; Must not belong to an HMO or a PPO, nor have private insurance nor benefits through the Department of Veterans Affairs; Must not have ben seen by an ophthalmologist in three or more years; Must not have been seen by an EyeCare America volunteer ophthalmologist in the past

Visit website for more information and online referral questionnaire; if eligible, a letter will be mailed in 1-2 weeks with the doctor's information

Fees

Medicare and other insurance will be billed but accepted as payment in full for the physician's services waiving co-pays; Uninsured are seen at no charge
