Northgate Care Facility is a 24 hour skilled nursing facility offering: Physical, occupational, and speech therapy with licensed therapists 5 days a week; Rehab-to-home services; Sub-acute care (IV, tracheotomy care, feeding tubes, wound care); Restorative program; Memory care; Hospice care; Respite care; Adult day program; Home care (non-medical personal care and companionship); Free transportation, and Access to a licensed dietitian.
