Provides the following services to those with special needs in the community:

Day Habilitation

Supported community living promotes skill building services that are unique and individualized to each participant.

Supported employment promotes skill building services in paid work environments.

Supported education provided to individuals enrolled in post-secondary institutions or other employer recognized certificate program. Including HiSET (formally GED), career exploration and planning, assistance in obtaining tuition and related expenses, application process, study strategies, assistance in obtaining tutoring and accommodations, and other supports needed that interfere with educational success.

Supported community living for people with disabilities. Children and adults learn things like how to balance a checkbook, brush their teeth or set a shower schedule, make new friends, be safe in their community, communicate with others, etc. The goal is for people to achieve their own goals of independence. Helps individuals with developmental disabilities, intellectual disabilities, physical disabilities, mental health challenges, and acquired brain injuries live as independently as possible in the communities of their choice. The individuals pay their own rent and utilities and for their own daily expenses.

WAIVER HOME SERVICES (GROUP HOME) | FAMILY RESOURCE CENTER

A waiver home is an alternative to institutions for the elderly and people with behavioral or mental conditions/disabilities. Staff is trained to help each consumer understand that living in a waiver home means that they are required to be considerate of others and respectful of the rules in their homes. Staff also help them understand that living in a waiver home is not a final chapter where someone "put" them, but rather, a new chapter where they can function to the best of their abilities with trained staff members who are aware of each individual's circumstances. The group home situation allows the individual to develop skills in:

- Financial management

- Cooking

- Cleaning

- Shopping.

An individual can also learn more about personal relationships and connecting to their community.

SUPPORTED LIVING SERVICES FOR ADULTS WITH DISABILITIES | LUTHERAN SERVICES IN IOWA (LSI)

Offers a goal oriented service in which a provider works one-on-one with an individual with disabilities to teach and assist with life skills. Services can be provided in the individual's home or in a community setting.

SUPPORTED RESIDENTIAL SERVICES | MOSAIC

Intermittent services to maintain or increase independent living skills. For people who live in their own apartments, staff provide personalized supports including assistance with banking, grocery shopping, medical appointments and accessing their communities. The amount of support is guided in partnership with staff and is documented in the Individual Program Plan. Also provides respite services as an additional support to families. Typically, people served are eligible for Supported Residential Services if they do not require staffing 24 hours a day, seven days a week.

ST. ANNE RESIDENCE | CATHOLIC CHARITIES - DIOCESE OF SPRINGFIELD

An affordable senior living facility that has 23 one-bedroom apartments complete with a full kitchen (appliances included) and bathroom. This is an independent-living community and is wheelchair accessible. A community room provides an accessible area for activities, events, and socializing. A central laundry room is available, along with a beautiful library and an elevator for 2nd floor access.

Offers two levels of Supported Community Living Services designed to meet the daily living needs of adults with disabilities and mental illness. Offers supported community living and respite services for persons with mental illness, developmental disabilities, or physical disabilities to assist them in living, learning, working, and socializing as independently as possible in their communities. Social opportunities, medical and social transportation, and in-home care (daily living skills) are provided.

Respite care both in and out of the home for adults and children is available with prior authorization.

Provides day and respite services, and addresses individual needs and desires for care, for informed choices and the individual's vision for the future. OWL is an approved provider for the Brain Injury Waiver Program, which was created by Iowa State Legislature in 1996. Services provided through the program include, supported Community Living-designed to assist the consumer with daily living needs, which includes, but are not limited to, community skills, personal needs, budgeting, social skills, home management and individual advocacy.

OUTPATIENT SERVICES | CENTERPOINTE, INC.

Adult treatment offers outpatient treatment services for adults 19 and over with mental health disorders or co-occurring substance use and mental health disorders. Services are provided based on client need and may include individual, group, and family counseling and psychiatric services. Expected stay depends on client need, but generally 6-8 months.

Adult Day Rehabilitation Services (MidPointe) provides long term, structured day services for adults 19 and over with a severe and persistent mental illness or co-occurring mental and substance abuse. Services develop daily living skills designed to reduce admissions to more intensive services. Expected stay up to 5 years.

Community Support Program offers individual support and advocacy to develop skills needed to live in the community as independently as possible. Services assist with basic needs such as housing, food, medical needs and linkages with needed services in the community. Expected stay varies depending on client needs, but generally, 12-18 months.

Youth services.

Recovery Support provides case management and crisis/relapse prevention for adults experiencing mental illness, substance, or co-occurring disorders and who need minimal professional contact to reside independently in the community.

SOAR is SSI/SSDI Outreach Access and Recovery. This program provides technical assistance and support with disability benefit and other mainstream resource applications for individuals who are experiencing homelessness and have a serious mental illness.
