Copper Creek provides a secured, assisted living residence and support services for adults with memory loss, including Alzheimer's disease and dementia. There are 10 units with 24 hour building security; 3 meals per day plus snacks in restaurant style dining room; social, cultural, spiritual, and educational programs; personal care and wellness services; medication management assistance; personal emergency response system; regularly scheduled transportation services; bathrooms, housekeeping, laundry services, air conditioning; electric heat and water included in rent; cable television and telephone service from local vendors.

Assisted residential services provides structured supervision, evaluation, and training, independent living and social/recreational skills as prescribed by an Individual Program Plan (IPP). Homes are located in residential neighborhoods where clients are active members of neighborhoods and communities. Staff is available to provide 24-hour supervision and training if needed.

People who apply to live in Mosaic housing do not have to receive other services from Mosaic and may receive services from other providers. Provides information and assistance to older adults at several sites in the community. Seniors needing assistance with Medicare, housing, energy assistance, Public Aid or other needs are able to meet with and receive assistance from a certified caseworker. Seniors may call to speak with a caseworker or meet with a caseworker in their own home if they unable to come into one of the sites affiliated with Ogle County SIS.

Single-level, private residences in Lincoln that are licensed, staffed, and equipped to provide long-term care with private rooms and baths in a neighborhood setting.

DS services include a Mon-Fri Community Day Service Program, Community Support Services (including CILA and Respite), and Supported Employment Services. Supports provided by staff are based upon each individual's needs, preferences, and strengths.

Also provides a Children's Home Based program for individuals from birth to age 18 with a Developmental Disability. Services included are behavior intervention and treatment, service facilitation, and personal support.

SCL services are intended to provide for the daily living needs of the member and shall be available on an as needed basis up to 24 hours per day.

ADULT COMMUNITY SERVICES program for adults with autism uses an individualized treatment plan based on the client's needs. Services can range from intensive 24-hour supervision to 1-2 hours of support per week. Accept Medicaid waivers, habilitation, state/county, and private pay.

Intermittent services to maintain or increase independent living skills. For people who live in their own apartments, staff provide personalized supports including assistance with banking, grocery shopping, medical appointments and accessing their communities. The amount of support is guided in partnership with staff and is documented in the Individual Program Plan. Also provides respite services as an additional support to families. Typically, people served are eligible for Supported Residential Services if they do not require staffing 24 hours a day, seven days a week.
