Offers two levels of Supported Community Living Services designed to meet the daily living needs of adults with disabilities and mental illness. These services increase a client's independence and provide a level of relief for family members and caretakers. LIVING SERVICES FOR ADULTS WITH DISABILITIES | CHRISTIAN OPPORTUNITY CENTER

HCBS (HOME AND COMMUNITY BASED SERVICES) programs provide training and support to people living in shared homes in the community with opportunities for learning skills to help develop independence. (Pella/Knoxville and Indianola/Des Moines regions).

SPIRITUAL SERVICES are available to all clients.

HOUSING SERVICES | CENTERPOINTE, INC.

Shelter Plus Care is project based rental assistance for single adults who are chronically homeless and experiencing problems with serious mental illness, substance dependence, or co-occurring disorders.

Supportive Living is housing for adults and families who are experiencing problems with serious mental illness or co-occurring disorders with priority for individuals transitioning out of the Lincoln Regional Center, Crisis Center, or other emergency levels of care.

Overland Trail Apartments provide Section 811 housing assistance in one bedroom apartments for adults experiencing problems with serious mental illness or co-occurring disorders.

Transitions provides independent living apartments and case management for transition age young adults, 18-24, who are experiencing homelessness and problems with substance abuse, mental illness, or co-occurring disorders.

Transitions II provides rental assistance for small families where a parent is experiencing homelessness and problems with serious mental illness, substance abuse, or co-occurring disorder.

Glide transitional housing in scattered site apartments in addition to case management services is provided for single, homeless adults over 19 years of age who have substance use and mental health disorders.

Permanent housing is a housing and case management service for single, homeless adults over 19 years of age who have substance use and mental health disorders.

Assisted residential services provides structured supervision, evaluation, and training, independent living and social/recreational skills as prescribed by an Individual Program Plan (IPP). Homes are located in residential neighborhoods where clients are active members of neighborhoods and communities. Staff is available to provide 24-hour supervision and training if needed.

Provides quality, affordable and accessible housing to people with intellectual and developmental disabilities. People who apply to live in Mosaic housing do not have to receive other services from Mosaic and may receive services from other providers. All residents must qualify according to established income guidelines. Income must be below the low or very low-income guidelines for the county of residence.

Community-based services and support to persons of all ages with developmental disabilities.

Residential program includes staffed group homes for youth and adults, supervised apartments, and independent living arrangements.

Vocational training and employment services through a supervised training center (HUB), which includes on-site contract work, work stations in industry, supervised employment programs, and job coaching. Additionally, High Plains Employment Services concentrates on job readiness and securing employment.

Year-round educational program is available to children through contractual arrangements with the school district.

Intergenerational Center, 1406 14th Ave, for individuals who have reached retirement age. Clients are served through an alternative day program that emphasizes recreation, leisure, and maintenance of daily living skills. Offers assisted residential living for individuals with an intellectual and developmental disabilities. Each home is adapted and personalized to meet each individual's needs, as well as their physical and emotional needs. Staff will provide as needed training and assistance with all aspects of living independently. Including cooking and cleaning skills, transportation, medical needs, making and keeping appointments, grocery shopping, paying bills, and more.

This program allows individuals to learn, grow and show independence and more self-sufficiency while working and building a life. Residents have their own lease while still receiving support as needed through the Friendship House.
