Provides comprehensive health care coverage to low income adults. It is an Iowa Medicaid program that covers adults ages 19-64 with income at or below 100% of the Federal Poverty Level. A Medicaid Waiver that provides healthcare coverage for children in families that meet the income eligibility requirements but are not eligible for Iowa Medicaid, and do not have access to other health insurance coverage. Covered services may include: doctor visits, inpatient and outpatient hospital care, well child visits, immunizations, emergency care, prescriptions, eye exams and glasses, dental care, speech and physical therapy, ambulance, mental health care and substance abuse care. A "Dental Only" plan is also available for children who meet income eligibility but do not qualify for full-coverage Hawk-i because they have health insurance for medical care (but no dental care coverage).

SENIOR SERVICES | WEST CENTRAL NEBRASKA AREA AGENCY ON AGING

Senior Care Options program available to older Nebraskans who are considering admission to a nursing home and may be eligible for Medicaid assistance. Clients are screened according to a standardized assessment process developed by the Department of Health and Human Services.

Care Managers arrange for services, which allow older adults to live as independently as possible, for as long as possible.

Housekeeping and supportive services to include grocery shopping, cleaning, laundry, transportation and personal care.

Family Caregiver Support for those caring for an aging adult or grandparents caring for a minor child. Provides information, assistance, support group, respite care, and supplemental services.

Home delivered meals are provided to homebound by reason of illness, disability or isolation. The meals meet 1/3 of the daily dietary recommended needs of adults 60 years and over and the requirements of the Older Americans Act and state and local laws.

Public benefit services, legal assistance, education, Medicare filing, and volunteer training through SHIIP.

MEDICAID WAIVER SERVICES | LEAGUE OF HUMAN DIGNITY

Medicaid Waiver services coordination for adults with disabilities determines eligibility for consumers, service providers, case management, and Medicaid Waiver services. Assists consumers to develop a plan for in-home services to help eligible consumers to remain in their home; and coordinates needed services. Persons must be Medicaid eligible and meet nursing facility level of care and have care needs which could be met through waiver services at a cost that does not exceed the cap. In addition, persons must have received an explanation of nursing facility services and waiver services, and elect to receive the waiver services.

Provides information and assistance to older adults at several sites in the community. Seniors needing assistance with Medicare, housing, energy assistance, Public Aid or other needs are able to meet with and receive assistance from a certified caseworker. Seniors may call to speak with a caseworker or meet with a caseworker in their own home if they unable to come into one of the sites affiliated with Ogle County SIS.
