Educational follow-up, care coordination, management and peer education, including care for sexually transmitted diseases, latent tuberculosis, high lead levels, prenatal care, and care for chronic diseases (asthma, diabetes, hypertension, high cholesterol, and depression).

Specialist referrals for services not available at the clinic are made for all patients. Appointments for patients without insurance and/or limited income are made through Hope Medical Outreach Coalition volunteer physicians and facilities.

Helps support the healthy development of babies during pregnancy by identifying risks and providing a support system. The Nine2Thrive project model is a public - private partnership bridging primary care and public health services.

Through screenings, prenatal healthcare providers refer pregnant individuals to Nine2Thrive for free, confidential connection to support. Individuals can refer themselves to the Nine2Thrive program, but their primary OB Physician must be in Polk County or the South Central Iowa Medical Clinic in Corydon. Support Specialists from Nine2Thrive link pregnant individuals to community and healthcare resources. The Nine2Thrive Support Specialists follow up with the referring healthcare provider.
Community supports individuals may be linked to include: mental health supports, financial assistance, parenting support & education, child development information, other individualized supports to meet the family's needs.
Fayette County Public Health offers basic health and dental screening; prenatal care, and well child care education. Evaluations of family's social and financial needs will help determine appropriate referrals for needed services.

Healthy Pregnancy Program services may be offered to clients during WIC appointments, in their home, or at a local public health agency. Clients can receive Healthy Pregnancy Program services in addition to other programs, such as local home visiting programs. Healthy Pregnancy Program home visits are done by a nurse who can do a physical assessment and check on the baby.

Helps to ensure the health of children, birth to 21 years of age in Appanoose, Clarke, Decatur, Jasper, Lucas, Marion, Monroe, Poweshiek, Ringgold, Wayne Counties. Services include help locating a doctor or dentist, finding health care coverage -Title XIX or Hawk-i, reminders for child/children medical and dental checkups, making doctor appointments, help to find community resources, health education and support, oral health services, help with payments for physical and dental exams, blood lead testing, immunizations 0-6 years, 7-18 years and adult immunizations.

HHH offers professional caregivers to provide prenatal and postnatal assistance.

Services include post-operative C-section care, light housekeeping, laundry, cooking, caring for other children in the home, infant care, and companionship.

Affordable, comprehensive primary health care especially for those with limited resources. Includes general physical exams and immunizations.

Services are provided regardless of ability to pay through a sliding fee discount program. Fees are calculated based on household size and documented annual income. All patients are eligible to apply for the sliding fee discount, even those with insurance.

Dental Clinic offers preventative dental services including: X-rays, exams, cleanings, fillings and extractions.

Pharmacy services offered to patients. Those eligible for the sliding fee program receive their medications at 30 to 50% below the retail market. Kohl's, Walgreens, Genoa, and Walmart fill all medications. Mobile Medical Clinic services, in which a van visits different locations to see patients.

HIV/STD testing provides confidential HIV/AIDS testing and counseling.

Transportation program provides a limited number of cab vouchers.

Mental health services are provided in-house through a partnership with Lutheran Family Services of Nebraska, as well as being referred out to collaborating behavioral health agencies.

Prenatal care for women.

Radiology has certified x-ray technicians to provide limited x-ray services.

Medication assistance programs help uninsured and underinsured patients apply to various pharmaceutical companies. Staff complete applications for patients to obtain their medication at an affordable cost.

Translation services offered to non-English speaking patients. Languages include Spanish, Vietnamese, Arabic, Kurdish, Burmese, Karen, and Thai.
