Childhood and adult vaccines for all people are provided at each of Elkhorn Logan Valley Public Health Department's three locations in Norfolk, Tekamah and Wisner. Includes vaccines for those who are privately insured, Medicare, Medicaid and those who are uninsured.

Childhood immunizations, flu shots, education and home visits to teen moms, cancer screening on site, child psychologist on site, breastfeeding assistance, smoking cessation for pregnant women, TB treatment, help finding a doctor or dentist, help with finding medication or other care, Every Women Matters Education and Outreach, Denver Developmental Screening, and case management to reduce lead and other hazard exposure.

Disease surveillance, investigation, and control of influenza, food-borne illness, whooping cough, flu, Fifth's disease, lice, MRSA, etc. Education and recommendations to schools, child care facilities and communities at large regarding disease prevention and control.

Provides childhood, adolescent, and adult immunizations to those in need who live in Hardin or Grundy County. Flu vaccines also available.

Maintains, promotes, and restores health through caring and collaborations with other health professionals. Services include communicable disease control and prevention, immunizations, flu shots and blood pressure screenings.

Medical and behavioral health care for all ages.

Services include chronic and acute illness exams, well child checks, school and employment physicals, well woman exams, chronic care management, behavioral health therapy, immunizations, hearing tests, DOT physicals, CDL exams, and more.

Telehealth services available for both medical and behavioral health.

Refugee Assessment and Immunization Clinic provides the required medical assessment and vaccines to newly arrived refugees of all ages within the first 90 days of arrival in the United States. Referrals to private medical providers for ongoing services are made. This program works in collaboration with local refugee resettlement agencies.

Follow-up visits for patients with diabetes, elevated blood lead levels, hypertension, behavioral health concerns, and other on-going health problems are also routine activities at the clinic.

Health education is integrated into each patient's visit. Nutrition, disease prevention and management are taught to patients individually and in groups. Educational follow-up, care coordination, management and peer education, including care for sexually transmitted diseases, latent tuberculosis, high lead levels, prenatal care, and care for chronic diseases (asthma, diabetes, hypertension, high cholesterol, and depression).

Specialist referrals for services not available at the clinic are made for all patients. Appointments for patients without insurance and/or limited income are made through Hope Medical Outreach Coalition volunteer physicians and facilities.

The team also provides treatment for chronic health conditions like diabetes, cardiovascular disease and asthma.

Offers public health programs and services and works closely with the county Board of Health. Also provides information about communicable diseases.
