Home Health typically includes skilled nursing, therapy (physical, occupational, or speech), or assistance with person care tasks such as bathing or dressing.

Participants receive all the care and services covered by Medicare and Medicaid, as authorized by the Interdisciplinary Team, as well as additional medically necessary care and services not covered by Medicare and Medicaid.

Provides in home personal care for persons with developmental disabilities including meal preparation, personal finances aid, personal hygiene, leisure and recreational skills.

Provides personal care, homemaker services, help with the activities of daily living, companionship, and respite care.

Non-medical in home care services including medication reminders, bathing, toileting, dressing, homecare-light housekeeping, meal prep, errands, and companion care for socialization and engagement.

Provides ambulation assistance, bathing and showering, continence care and toileting, skin and hair care, dressing, feeding, nursing services such as medication set-up, glucose monitoring, wound care, and taking vitals.

Helps senior citizens, who might otherwise need nursing home care, to remain in their own homes by providing in-home and community-based services. Services include:

- Comprehensive care coordination.

- Adult day services.

- In-home services (cleaning, meals, laundry, shopping/running errands, and personal hygiene).

- Emergency home response service.

- Automated medication dispenser service.

Provides help to senior citizens, who might otherwise need nursing home care, to remain in their own homes by providing in-home and community-based services. Personal care services including assistance with bathing, grooming, dressing, eating, mobility, medication reminders, etc.

Home care services, including light housekeeping, laundry, changing linens, meal preparation, etc.

Care coordination advocacy program to create doctor appointments, collaborate with families, help transitions, health care portfolios, etc.

Home health coordination to hospice, home health, etc.

Medicaid Waiver approved - works with ENOA, VA, and League of Human Dignity.

Provides services to individuals who have conditions such as congestive heart disease, kidney disease, diabetes, muscle-nerve problems or respiratory diseases and are able to be discharged from a hospital or nursing home but need additional care at home. Require short term assistance at home because of same day or outpatient surgery or maternity-related incapacity.
